Nail fungus (onychomycosis) is considered an aesthetic problem. Moreover, there is an opinion that over time it can pass on its own. However, this is a big - and dangerous - fallacy. Fungal infection not only does not go away on its own, but gradually "assimilates" all new parts of the body. Doctors-dermatologists note: up to 50% of patients come to them with advanced cases of onychomycosis, when simple remedies for nail fungus do not help and serious therapy with many possible "side effects" cannot be dispensed with [1].

Who is at risk? Statistics say: among patients with onychomycosis, there are much more residents of megalopolises than people living in rural areas. By the way, men suffer from nail fungus one and a half to three times more often than women, but they visit a doctor less often [2]. The type of activity is also important. Often, fungal infection of the nails occurs in workers of industrial enterprises and coal mines, technical personnel of nuclear power plants. The reason lies in the presence of harmful factors in these industries - this is the need to wear rubber shoes in conditions of high temperature, dust and gas pollution, the presence of ionizing radiation, and so on. Athletes and military personnel are also at risk. The use of shared showers and exercise equipment increases the risk of infection. A separate category is employees of baths, showers, laundries, service personnel of medical institutions and rest homes (masseurs, orthopedists) who come into contact with a large number of possible carriers of fungal infection [3]. Although infection with a fungal infection can occur at any age, older people face it more often due to weakened immunity [4].

In fact, literally everyone is "in danger": sometimes the disease occurs against the background of stress, taking antibiotics, and even a small wound can help the penetration of infection. In addition, if one of the family members is a carrier of the fungus, it is most likely that other household members cannot avoid infection: pathogenic microorganisms can pass to them when using shared towels, slippers, bed linen.

To avoid unpleasant consequences, it is extremely important to start therapy on time. Of course, the prescription of the medicine remains with the doctor, but it is useful for everyone to know about the types and characteristics of medicines for nail fungus.

How fungal remedies work

Antifungal nail agents can be local or systemic. The latter are "heavy artillery", since along with high efficiency they have a large number of side effects and contraindications. For this reason, antifungal pills and capsules are usually available from pharmacies with a prescription.

Local remedies against nail fungus include all kinds of solutions, sprays, creams, varnishes. The main advantage of local therapy is that the drug practically does not penetrate into the systemic circulation. This avoids the side effects associated with oral antifungal drugs.

However, not all topical preparations are equally effective. So, the active substance in varnishes is contained in a minimum concentration, moreover, it cannot penetrate deep into the nail plate. That is why the "curative coating" is relevant only for superficial onychomycosis and only if the affected area is less than a third of the nail. As for sprays, ointments and creams, they can be applied at different stages of the fungus. Nevertheless, despite the fact that onychomycosis is often included in the list of indications, such agents are more suitable for the treatment of skin mycoses. It is better to choose drugs in liquid form - solutions - the drug of these drugs can penetrate deep into the nail plate.

However, not all solutions are equally effective. Why does this happen if the medicine is able to "get" to the very focus of the infection? The fact is that there are many causative agents of fungal infection - these are dermatophytes, molds, and yeasts ... Some drugs can really fight different types of infection equally well, but others are active only against certain pathogens. Therefore, it is necessary to pay attention to the composition of the preparation. Let us dwell in more detail on what active substances can be in antifungal agents.

This remedy for the treatment of nail fungus, which was discovered more than 45 years ago, has been comprehensively studied and is successfully used to this day for the external therapy of mixed fungal infections of both nails and skin. It has an effect against fungi and dermatophyte spores, as well as mold, yeast and yeast-like fungi.

In addition, naftifine has an antibacterial effect, comparable in effectiveness to that of an antibiotic [6], helps to reduce the manifestations of inflammation, and soothes itching.

This substance is used for topical therapy of fungus caused by yeasts and dermatophytes. Miconazole has a limited antibacterial effect, to which only gram-positive microorganisms are sensitive. In this regard, it is customary to combine it with other medicinal substances. For example, with metronidazole, which suppresses other types of bacteria and protozoa, as well as with lidocaine, which performs the function of pain relief [8].

Onychomycosis, aka nail fungus, is a fairly common disease. According to various sources, it affects from 2 to 25% of the world's population [1]. Onychomycosis of toenails and nails can cause pathogens - dermatophytes, as well as yeast and mold fungi. All of them are quite tenacious and can freely "travel" through the body and be transmitted through common clothing or household items. In order not to infect colleagues and family, the fungus must be quickly dealt with. We will talk about methods of treating onychomycosis in the article.

How to recognize onychomycosis: symptoms that are easy to overlook

Nail fungus appears gradually: as the plate is affected, minor changes give way to more obvious and painful signs. Let us examine in more detail the most famous symptoms of onychomycosis of toenails and hands.

  • Itching and tingling. These are some of the first signs of a fungus that many tend to ignore. At first, itching and tingling may not appear constantly, but, for example, with physical exertion or contact with water. However, then they begin to intensify and transform into a burning sensation that follows the carrier of the nail fungus almost continuously.
  • Layering. With a fungus, even the strongest and healthiest nails become thin, brittle and begin to crumble. If measures are not taken in time, the entire plate may peel off, which is very dangerous. Firstly, being left without a nail is very painful, because there are many nerve endings at the fingertips. Secondly, various infections can get into an open wound and aggravate the discomfort. And thirdly, the plate will take a long time to recover, which also does not make the situation easier.
  • Change nail color. With onychomycosis in children and adults, the color of the nail plate often changes. A healthy pale pink shade of the nail becomes yellowish, brownish and even grayish. This is striking and brings at least aesthetic discomfort.

In addition, with onychomycosis around the nail plate, skin inflammation is often observed. As we have already said, pathogenic fungi are tenacious and can easily infect areas adjacent to the foci. Skin fungus - mycosis - is less common than onychomycosis of the nails of the hands and feet, but often proceeds in parallel with it. With mycosis, the inflamed areas of the skin begin to itch, pulsate and sometimes swell. Periodically, scales, blisters and small ulcers are also formed on the affected surface. In this case, complex therapy is important, otherwise pathogens will "travel" back and forth.

Almost every person can develop a fungus, but there are also risk groups. Diabetics and people who have recently suffered a serious illness need to be especially careful, since a weakened immune system can surrender under the pressure of onychomycosis. Injured skin or nails can become a magnet for fungus. Disruption of blood supply to fingers and toes, usually seen in overweight and inactive people, increases the risk of developing the disease. Wearing tight shoes can also cause nail fungus - a warm, humid environment encourages fungal infections.

We should also mention the elderly, in whom the fungus occurs several times more often than in the younger population [2]. This is due to a decrease in the growth rate of the nail plates, weakening of the ligaments, stagnation of salts and other age-related changes.

Treatment for toenail and hand nail fungus

Therapy for onychomycosis is necessary. This is a full-fledged disease that, if inactive, does not go away with time and almost always requires medication. Before starting the treatment of onychomycosis, it is imperative to consult a specialist. It can be a dermatologist or a doctor of narrow specialization - a mycologist. The doctor will help you find out the nature of the fungus and choose the most appropriate therapy based on the stage of the disease.

However, it is no secret that many of our compatriots are afraid of people in white coats and try to delay meeting with them to the last. Including with the help of various folk remedies. On request "treatment of nail onychomycosis" the Internet gives out many recipes. This is a broth of celandine, and compresses with iodine, and baths with potassium permanganate. But, if you study such recipes carefully, it becomes clear that they do not offer a single treatment option for onychomycosis of the nails and mycosis of the feet and hands. Everywhere, different recommendations for use and dosage are indicated, not to mention the fact that such alternative methods have not undergone clinical trials, which means that they have no proven effectiveness. Judging by the numerous reviews, home methods can only help at an early stage of the fungus, and even then not always. At the same time, a large concentration of iodine or potassium permanganate can cause a burn and only aggravate the situation. Therefore, for the treatment of onychomycosis of toenails and hands, it is worth giving preference to pharmacological agents prescribed by a doctor.
